List of forms and documents to enclose with your official immigration application

IMPORTANT :

- Answer all questions, adding the apartment number to your address where applicable. Indicate “n/a” (not applicable) to questions that do not apply to your case.


- Read the instructions carefully, paying special attention to the definitions before filling out the forms. Incomplete forms will be returned, delaying the processing of your file.


- All copies enclosed with your application must be authenticated by the institution which holds the original or by the legal authority duly authorized to certify as a true the copy of a document, that is to say a copy confirming the existence of a valid original document.
- The documents must be accompanied by an official translation in French or English if they are in another language.

 
- The originals must be available on request or be presented during the interview, if any.


- If you cannot provide one or more of the requested documents, identify it and explain the reasons. You may also enclose other relevant documents.
- The truthfulness of the information and documents enclosed with your application may be checked at each step of the selection process. A false declaration may result in your applica-tion being rejected.

 

1. Application for a Québec Selection Certificate

  • The principal applicant and his or her spouse or de facto spouse must complete the form entitled Application for Selection Certificate - Principal applicant, spouse or de facto spouse (Dynamic PDF, 599 Kb).
  • Each child aged 22 or older who is accompanying you must complete the form entitled Application for Selection Certificate - Dependent child aged 22 or older or who is married or a de facto spouse (PDF, 133 Kb).
  • Staple an identifying photograph of each person (adults and children) in the Photos section of the principal applicant’s application.
  • Where applicable, spouses must complete the appendix Declaration by spouse or the facto spouse (PDF, 56 Kb).
  • The principal applicant must fill out the Contract respecting financial self-sufficiency (PDF, 64 Kb).
  • For each dependent aged 22 or older who is accompanying you, you must enclose:
    • proof that the person is still celibate (certificate of celibacy) or
      proof that the person married or became a de facto spouse before age 22 (act of marriage);
    • proof that the person has been studying full time without interruption since age 22, by enclosing a certified true copy of the original academic transcript for each year of study since age 22 or, if the person married or became a de facto spouse before age 22, since the marriage or
      proof that the person is dependant on the principal applicant, by enclosing a certified true copy of the original medical certificate attesting to the physical or mental incapacity of the person.

  • If you are represented by an immigration intermediary, enclose an original power of attorney signed by you and your intermediary.

5. Curriculum vitæ

  • Enclose your curriculum vitæ (CV) and that of your spouse, if any.

6. Studies and training

  • Where applicable, enclose the following documents for yourself and your spouse or de facto spouse:
    • the copies certified as true to the originals by the issuing educational institutions of all diplomas and certificates (excluding those pertaining to primary studies) arranged and stapled in descending chronological order;
    • the certified true copy of the translation of the diploma, if the original does not exist in French or English;
    • the copies certified as true to the originals by the issuing educational institutions of all transcripts for the final year of each level of studies (secondary, postsecondary or university);
    • the certified true copy of the translation of the academic transcript, if the original does not exist in French or English.

7. Proficiency in French or English

  • To evaluate your language knowledge if neither French nor English is your mother tongue, you can enclose documentary proof attesting to your proficiency in French or English and, if applicable, that of your spouse.
  • As of January 2007, you may demonstrate your French language knowledge by presenting the MICC with the results of a French evaluation test adapted to meet Québec’s needs and given by one of the two organizations recognized by the MICC, which are:
  • To establish your English language knowledge, please enclose with your application the necessary documentary proof for yourself and your spouse, where applicable. This proof could be the results of a recognized evaluation test, such as IELTS, TOEFL, TOEIC and Cambridge tests.
  • You may also enclose a document to prove that you have worked in English or French.


  • Note: You must enclose original documents and your documents must include an assessment of comprehension and oral expression.
    It is important to note that the level of French proficiency will be assessed based on proof submit-ted. Consequently, it is preferable to enclose all proof that demonstrates your level of French proficiency.

8. Work experience acquired over the last five years

  • Enclose with your application:
    • All your work certificates for the last five years for you and your spouse, where applicable. These certificates must be originals;
    • Documentary proof that your work experience was acquired legally: record of your contribu-tions to social security (CNSS, CNAS or equivalent) or proof of exemption from paying social security premiums. The documents furnished must be originals issued by the agency;
    • For work internships: enclose the attestations from the employer or establishment with proof that the internships were remunerated and/or performed in the framework of your studies.
  • The work certificates must be produced on the company’s letterhead. They must be dated and signed and bear the signatory’s name. They must also give the following information:
    • start and end dates of your employment;
    • your social security registration number;
    • contact information for the employer, including telephone number;
    • the job positions that you occupied;
    • a brief description of the tasks you performed.
  • Self-employed applicants must enclose with their application:
    • the copy certified as true by the issuing institution of the trade register;
    • proof of payment of income tax over the last five years, or proof of exemption, where applicable;
    • any other proof of the operation and legality of the business (history of your contributions to the social security pension fund for non-salaried workers or proof of exemption from paying social security premiums). The documents furnished must be originals issued by the agency.

9. Stay in Québec

  • Enclose proof of your stay in Québec with your application (certified true copy of the original visas or residence permits or photocopies of appropriate passport pages) and state the length of your stay.

10. Family relationships in Québec

  • If you have a family member residing in Québec (spouse, de facto spouse, father, mother, son, daughter, brother, sister, grandfather or grandmother), attach the following to your Declaration of a relative in Québec (PDF, 47 Kb):
    • a certified true copy of the original of his or her permanent residence card, Canadian passport or Canadian citizenship card;
    • a copy of his or her latest notice of assessment for provincial income tax, or a copy of a lease (in the case of a tenant) or copy of a property tax statement (in the case of an owner);
    • a copy of the document proving your family tie to this person (official family record book (livret de famille), acts of birth or marriage, etc.).

  • Enclose with your application:
    • the certified true copy of the original of your birth certificate and that of each person accompanying you, your official family record book (carnet de famille), where applicable, and photocopies of all pages of your passports bearing entries;
    • the certified true copy of the original of your marriage certificate if you are married or, if you are a de facto spouse, the official written proof of your cohabitation for at least one year (cohabitation certificate (certificat de concubinage) certified true copy of the lease, bank accounts, electricity and telephone bills, etc);
    • the certified true copy of the original of the divorce judgment, where applicable;
    • the certified true copy of the original death certificate of the spouse, where applicable.
